Conference Attendee Registration Packages

Single Occupancy- $565
Double Occupancy- $485 (two attendees per room/each paying $485. Double Occupancy must register at the same time)

No lodging required registration- $295
• Includes all items listed below but does not include breakfast or overnight stay

Registration Costs Includes:
•     Sunday/Monday overnight lodging,
•     Dinner Sunday,
•     Breakfast, lunch, dinner, break refreshments on Monday,
•     Breakfast, lunch, break refreshments on Tuesday

•     July 1, 2012 – June 30, 2013 Membership Dues DAO and/or OSROA (depending on your status as an SRO or a DARE Officer)
•     Conference T-shirt
•     Door Prizes
•     Conference Training Materials

Spouse/Guest/Child Registration Packages
Family and guest package fees must be paid by June 21, 2012

There are two options for meals for children and spouses/guests:
1) Pay the bundle price for children and guests to eat with the conference group. Includes the meals only, not the break refreshments.
2) Bring the family, they will stay in your room (with Single Occupancy Registration only) and eat their meals on their own at the lodge restaurant or snack bar. There is no cost for this option.

Spouse/Guest Package: $140 Includes sharing room with paying registrant, scheduled meals with group (not coffee breaks or soda breaks). No charge if spouse/guest purchase and eat all meals on their own.

Child (12 & under) Package: $69 Includes sharing room with paying registrant, scheduled meals with group (not coffee breaks or soda breaks). No charge if child purchases and eats all meals on their own.

Please indicate on the registration form which option you choose. Family and guest bundle fees must be paid by June 21, 2012.

Please note: For planning purposes, registrations must be received by May 25, 2012. A late fee of $25 will be assessed if received after this date. Cancellations must be received no later than June 8,
2012. OSROA/DAO is financially committed to Maumee Bay Resort for all rooms on this date and no refunds will be issued.
